Improving Your Cancer Journey in Powys

If you; or a member of your family/ someone you care for have been affected by cancer; of if you are bereaved, we can provide information, advice, support, counselling, and complementary therapies.

You can be referred by a health professional, friend or self-refer into our service. Simply call The Trust on 01597 823646 or email: nurse@brackentrust.org.uk. We are open 9a.m. – 5p.m. Monday to Friday.

During the admission process, we use a specific tool (if required) called a Holistic Needs Assessment (HNA) which helps to identify your main concerns and then we agree a plan to support you with addressing those concerns. E.g., your current or future wellbeing including finances, travel, work, tiredness, anxiety and more…….
